  3. Logansport Memorial Hospital  Located in Logansport, IN  Founded in 1925  583 employees  83 Acute-care Inpatient beds  County-owned , Not-for-Profit , Independent hospital  Approximately 2,000 admissions per year  Cares for patients in Cass County and five other surrounding counties

  4. Why implement?  Promote patient-centered and family-centered care principles  Assist in developing strategies to help strengthen communication and collaboration between healthcare providers, patients, and their families  Improve patient and family satisfaction  Provide assistance in developing educational resources  Serve as a liaison between the community and the hospital

The Journey Begins… June 2012  Reviewing/Evaluating Patient Advisory Councils October 2012  Central Indiana Alliance for Health Hands-on and Intentional: Building Successful PACs November 2012  Identifying council responsibilities, obligations, and membership criteria  Begin recruitment of members

How did we get members?  Goal membership was 8 – 10 members  Requested recommendations from staff and physicians  Utilized individuals who had been able to articulate issues and wanted to help problem-solve  Identified key people in the community who could help be a liaison for the hospital  After receiving recommendations, individuals were notified to discuss the plan for this group and determine if he/she was interested

  8. Next Steps…  Solidifying individuals who were interested  Identifying a date/time of the first meeting  Identifying agenda items  Discussing methods of communication

  10. 2013 First Meeting  January 31, 2013  8 members − Specific responsibilities − Time obligations and commitment  6 meetings per year − HIPAA forms − Application − Press Ganey results

  11. 2013 Accomplishments  Inpatient welcome book  Signage throughout the hospital  Patient education handouts  Communication regarding test results − Still in progress

  12. After the First Year… What did we learn?  Needed to add hospital front-line staff to the committee  Our focus was too broad − Multiple departments discussed − Difficult to narrow scope − Need to separate into multiple Advisory Councils

  13. Recommendations  Patient Advisory Councils created  Inpatient/Admission  Physician Practices  Obstetrics  Emergency Department

  14. 2014 Action Plan  Member of the Patient Advisory Council added to the Quality Council  Implement Patient Advisory Councils for Physician Practices and Obstetrics  Increase membership to include managers and frontline staff

  15. Accomplishments- 2014 To Date  Joe Clothier has joined Quality Council  Jacinta Langley, RN, IBCLC and Laura Gallahan, MSN, RNC have implemented a Patient Advisory Council for Obstetrics − 11 members − Implementing support group − Assisting with a grant specifically focused on healthy mothers and babies

  16. Patient & Family Engagement “ It is impressive to me that Logansport Memorial Hospital is excited about including patient input as a necessary component of their Quality Improvement Philosophy. Asking members of the community to be part of the Patient Advisory Committee, and having a Patient Advocate Council Member as a member of the Quality Council can, with time, open up needed dialogue between patients and LMH. I believe this is a huge step towards improving the level of care given to patients in the Joe Clothier Logansport area.” Patient Advocate Council and Member of the Quality Council
