How did we get members?

  • Goal membership was 8 – 10 members
  • Requested recommendations from staff and physicians
  • Utilized individuals who had been able to articulate

issues and wanted to help problem-solve

  • Identified key people in the community who could help

be a liaison for the hospital

  • After receiving recommendations, individuals were

notified to discuss the plan for this group and determine if he/she was interested

slide-9

  • Solidifying individuals who were interested
  • Identifying a date/time of the first meeting

Patient & Family Engagement

“It is impressive to me that Logansport Memorial Hospital is excited about including patient input as a necessary component of their Quality Improvement Philosophy. Asking members of the community to be part

  • f the Patient Advisory Committee, and having

a Patient Advocate Council Member as a member of the Quality Council can, with time,

  • pen up needed dialogue between patients

and LMH. I believe this is a huge step towards improving the level of care given to patients in the Logansport area.”
