EXIT PASSES and ID

Students living within the vicinity of the school (within walking distance) and those allowed by their parents to travel home by carpool, DLSZ shuttle, or public transportation are encouraged to apply for an exit pass. Kindly submit the completely filled out EXIT/LUNCH PASS forms available at the Safety and Security Office (SSO) with your 1x1 ID pictures to the offices of the JHS Coordinators for Student Affairs and Formation (CSAF) on or before June 15, 2018 (Friday) to give the SSO ample time to print the school IDs. For those who need to update their ID photos, you may go straight to the SSO. The deadline of the ID photo capture is

• n June 15, 2018 (Friday) also.

Application for exit/lunch pass and photo capture resulting to printing new IDs after the set deadline shall have charges equivalent to the new ID fee.

EXCUSE LETTER FOR ABSENCES

A student who has been absent must present an excuse letter, with inclusive dates and reason for the absence, from his/her parents to the Coordinator for Student Affairs and Formation (CSAF) upon his/her return to school. A student who has been absent for three or more days due to illness must secure a clearance from the school physician prior to admission in class. The student must also submit a medical certificate with the inclusive dates of confinement and the reason for such, together with an excuse letter, to the class adviser and to the Coordinator for Student Affairs and Formation (CSAF).
