SIXTH FORM AGREEMENT
Sept 2020 – July 2022 In choosing to be a student in the Sixth Form at The Cotswold School, I agree to:
- Attend punctually all timetabled lessons, supervised study sessions, registration / tutorial sessions, assemblies and PSHE sessions.
- Work to the best of my ability, both in lessons and for independent study; to hand in set work on time.
- Remain on site for all independent learning periods (“free periods”) and make good use of these times to study.
- Always sign out when leaving the school site (e.g. at break or lunch time) and sign back in upon my return to school.
- Phone or e-mail the school (01451 812946 or sixthform@thecotswoldschool.co.uk) as early as possible on each day of absence due to illness.
- Not miss any school days due to holidays (including music festivals) and unavoidable appointments during school time. I will inform my tutor and the
Sixth Form Administrator of any planned but unavoidable absence by completing, in advance, a Request for Absence Form.
- Act as a positive role model for the younger students, which includes adhering to the School’s behaviour policy and no smoking and drugs policy.
- Follow the Sixth Form Dress Code.
- Take responsibility for all my belongings, including mobile phones (which can only be used in the Sixth Form Common Room) and take care of the
School’s environment, including not leaving any belongings in the Sixth Form Centre overnight.
- Recognise that the privilege of being a Cotswold School Sixth Former requires responsibility and commitment, and therefore I will participate as fully
as possible in school activities. To support me in achieving success in the Sixth Form I am entitled to receive:
- A programme of courses delivered by dedicated, professional teachers, using appropriate facilities and resources.
- A variety of activities and experiences in addition to academic study, including opportunities for spiritual, moral, social and cultural development.
- Careful monitoring of my progress, including: Ÿagreeing personal targets Ÿreceiving feedback from my teachers on the standard of my work Ÿ
receiving pastoral and academic support.
- Regular reviews of my progress and opportunities for my parents/guardians to come into school to discuss my progress.
- Advice and guidance on the routes I wish to follow beyond school, including a written reference for Higher Education or employment.
I understand that I am 100% responsible for my success, and that the school staff are 100% responsible for ensuring that my success is possible. SIXTH FORM ENROLMENT
(September 2020 – July 2022) NAME: ADDRESS: CORE OPTIONS: Notes: 1. Students who have not achieved a grade 4 in both GCSE Maths and GCSE English must continue to study these subjects until they have achieved grade 4 in both. This is now a legal requirement for all schools. We may also recommend that some students who achieve grade 4 in GCSE Maths or English (“standard pass”) should retake these subjects to try to achieve a minimum of grade 5 (“good pass”)
- 2. Any students who undertake only three core courses must fill their fourth option block with Supervised Study. (This includes, in Year 13, when most
students reduce their core options from four to three subjects.) I confirm that: I wish to enrol in the Cotswold School Sixth Form to study the core options listed above
- I have received clear advice and guidance on my suitability for these options, including information about the school’s entry requirements for these
courses.
- I will work hard to ensure that I successfully complete the above courses.
- I understand that to progress from Year 12 to Year 13 in a subject I must achieve a minimum of grade D in the AS level examination in that subject.
Or, for subjects without AS level, I must have made progress equivalent to grade D or above in Year 12 in that subject.
- I have read, understood and agree to the Sixth Form Agreement (overleaf).
- I understand that I am entitled to apply for a 16-19 Bursary (financial support for students from lower income households)
- I have the legal right to be a resident in the United Kingdom and will remain a resident of the UK for the duration of my full programme of study
(which, for most students, is two years). STUDENT’S SIGNATURE: ………....................……………………......... Date: ………….....………..

Getting to School
Bus Routes A & B are different - these routes are managed directly by Pulhams. Applications for bus passes on these routes are being accepted by Pulhams now: www.pulhamscoaches.com/home2school-2/
Sixth Form students are no longer eligible for a free home to school bus pass. It is therefore ESSENTIAL that you have a plan in place for how you are going to get to and from school.
Under normal circumstances GCC run the “Spare Seat Scheme”. This allows sixth form students to purchase a bus pass for any seats that are not allocated to lower school pupils on school buses. Due to social distancing guidelines, GCC will not be able to offer this in
- September. BUT, they are asking that if this would be your preferred method of home to school
transport that you still register so that if guidelines change which allow more students to travel
- n a bus they can contact you to offer a seat if possible.

££ ££ 16 16-19 years BURSARY FUND th the basics ££ ££
If your household income is £25,520 or less, you may be eligible - please apply. Forms can be downloaded from the school website and posted back or returned to sixthform@thecotswol dschoo.co.uk If you are eligible, the amount of money you receive will depend on the lump sum awarded to school and the number of students to apply. Eligibility is normally based on household income. Each year schools are allocated a lump sum
- f money to assist
students who
- therwise may not be
able to attend Sixth Form for financial reasons. Funds can be used to help towards costs related to full time education such as transport, meals, curriculum based trips, university open days/interviews, books and equipment This bursary is discretionary. Students in receipt of an award will have their attendance, timekeeping and conduct reviewed termly – 95% attendance is expected as a minimum.
