  6. Lessons Learned December 2012  Missed opportunities reported by provider offices  Phone calls instead of in person visits  Training materials dropped off instead of completing the training  Not having appropriate provider office employees present at the trainings 6

DHHS Action Steps • Restructured monthly meeting with the health plans • Created a communication forum for health plans to share training dates with state agencies • Developing better materials for smoother process  Simplifying referral form  Re-evaluating communication collateral for medical staff  Designing exam room posters • D & D sales team to meet with provider offices with highest incidence of low birth rate deliveries. Developing week of January 21st 7

DHHS Action Steps Continued Identify successful provider practices in screenings and brief intervention  Creating a shared step-by-step process/template based off of GHS and Dr. Gailey’s best practices  Obtaining appointments for Lunch and Learn for peer to peer consultation Presented SBIRT data to Medical Directors for MCOs & MHNs  Asked for 100% support  Stress importance of quality staff training  Commit to performing all screenings on pregnant & postpartum patients 8

Birth Outcomes Initiative  Bulletin Released in December for effective date January 1, 2013 for Non Payment Policy for non- medically necessary deliveries prior to 39 weeks  Race to the Date: DHHS created an incentive pool of $1,000,000 with a maximum payout of $200,000 to hospitals that achieve Baby-Friendly Hospital designation through Baby-Friendly USA by September 30, 2013  17 Letters of Intent received by hospitals to date  January 16 – introducing Phase 2 of Birth Outcomes Initiative

Retail Pharmacy Clinics  All 28 CVS Minute Clinics have been enrolled as Medicaid providers  21 of 90 individual Nurse Practitioners are enrolled and tied to group locations  Actively working with national leaders of CVS  to complete enrollments; reminder that electronic signature is allowed  Web Tool billing  Here, Not There Communications Strategy  Development of sales strategy for Wal-Mart and Walgreens for future enrollments as retail clinics  Phase 2: include “Retail Clinic” as a provider site on MMIS 15

  16. CHWs - Community Health Workers  Bulletin posted in December for primary care practices to attend informational meeting January 9 with rsvp required  Over 40 practices/organizations responded to the invitation  Approximately 100 people attended  The grant application and evaluation criteria were emailed directly to attendees on January 11th and communicated on DHHS website  Application deadline is Friday January 18 at noon 16

  17. CHWs - Community Health Workers  20 practices will be awarded the grant by March 1  Training for CHWs begins March 18  Midlands Technical College will implement a 6 week curriculum based on SC core competencies in four regional campuses  Working with Milliman to establish an incentive payment for MCOs who employ a CHW 17

Free Clinics  Statewide Free Clinic interview completed in December  Preliminary analysis internal due January 31  DHHS will present at South Carolina Free Clinic Association statewide meeting  Rosa Clark Free Medical Clinic to present Oconee Hybrid Model established in 2012 outcomes to DHHS in February

Governor’s Original Six Foundation DHHS Participation  Four Leadership Summits and Health Fair Events  Health and Job Fair previously held include: Allendale Marion Marlboro Bamberg  We are completing comparative analysis of health data on Bamberg vs. Marlboro  Upcoming Leadership Summit, Health and Job Fair scheduled in Barnwell County - March
