Michelle Clark, Municipal Advisor, Municipal & Property Division Step 1: “Form Templates” is a drop -down menu with various options where you will tell the system what type of document you are uploading. Step 2: “Notes” will allow you to enter a limited amount of text for reference purposes. This is optional and will be displayed in the “Notes” column of the Document Table you saw on the Home Screen after the upload has been completed. Page 8

The box will turn green, indicating that the system has validated the file. A message box will appear “File was uploaded successfully!” click “OK”. Note: This may take a few moments on slower connections and with larger files. Page 9
