‘t SPOOR An award-winning example of care coordination and comprehensiveness
SLIDE 5 Comprehensiveness
- Making sure the patients are provided with everything
they need
But also…
- Making sure the most vulnerable patients are provided
with the attention they need
SLIDE 6 (Care) Coordination
- The responsibility of primary care to arrange for services
that primary care is unable to provide such as social services, orientation services, etc
But also…
- The need for other services to integrate health and have
a holistic approach regarding the person
- The need for coordination and cooperation between
different levels and areas of governement
SLIDE 7 Patient-centered medical facilities in Belgium
Regional policy
- An alternative approach to medical care
- Providing care to people who have more
difficulties to access the healthcare system
- Multidisciplinary (doctors, nurses, social workers,
physiotherapists, dieticians, dentists, etc)
- Focus on prevention, health promotion and
participation of the “patientèle”
SLIDE 8 Patient-centered medical facilities in Belgium

- Family planning services (Wallonia)
- CAW (Flanders)
- Houses of the Child (Flanders)
SLIDE 10 The role of Public Social Welfare Centers in connexion with health (medical aid)
NON-URGENT
- Mainly for foreign nationals with
residence permit (asylum seekers, migrants granted residence on the basis
- f health motives)
- Allowing or renewing health coverage
- Medical card
- Agreements with health practitioners
- Paying for medication
URGENT
- For people without a legal residence
permit
- Not a financial aid
- Not technically “urgent”
- Must be administratively approved
by the PSWC
- This constitutes a barrier to
immediate health provision – we’re working on this with our project Mediprima (using computer-based technology to systematize decisions)
Federal policy – Local implementation

Looking to the future
Belgium has a high performing health system but there is always room for improvement Better coordination at all levels and between levels Good example: PPS SI and FPS Public health working together to implement the ROMED* programme (training intercultural mediators in hospitals to better respond to the Roma target population)
* A programme financed by the Council of Europe
SLIDE 17
Looking to the future
Belgium has a high performing health system but there is always room for improvement Aiming for comprehensiveness at the local level with systematic attention to those who are the most in need Good example: the Children First project supports local consultation platforms to prevent, detect and identify child poverty by putting all the relevant local professionals around the table (anti-poverty associations, child care centres, schools, sport clubs,…) in order to offer an all- round service offer to the children and families who benefit from the project
