! Our CDWs work with a range of communities who are less likely to access talking therapies ! We are especially keen to receive referrals for older
- adults. We have no upper age limit for referrals
! We work hand in hand with employment workers who can help patients to retain their current employment or gain new employment

Eligibility Criteria
Adults 18 years and above, living in Lewisham or registered with a Lewisham GP, experiencing: ! Depression or An Anxiety Disorder as the main problem ! Or other life event issues – for example interpersonal problems, relationship and family problems, adjustment to life events including complex bereavement ! Long term health conditions & medically unexplained symptoms

Not suitable for IAPT
! Patients with alcohol and/or drug dependence ! Patients with active CMHT or other secondary care service involvement ! Patients diagnosed with bipolar, personality disorder or psychosis ! Actively suicidal patients or people who repeatedly self- harm ! Patients requiring crisis intervention/treatment ! Patients where anger problems are the main concern

Stepped Care
! Stepped care ethos: we offer the least intensive, and most cost-effective, intervention that we think will be of benefit, and then step up as required ! Patients who are unlikely to benefit from our service will be directed to other services in a timely manner

How to access the service
! GP referral or self-referral, including online referrals via the service website ! Patients referred by their GP are required to opt in ! No opt in required for self referrals

What happens next?
! All referrals are processed at a single point of access ! Referrals for patients who opt in are screened and allocated to an appropriate therapist (low or high intensity) ! Patient is contacted to arrange the first appointment which is either a telephone triage appointment or a face to face assessment ! Patients who do not opt in within 2 weeks of being referred are discharged
