Physical Activity Referral Scheme • Enables health professionals to refer patients with a range of medical conditions. • Introduces participants to the benefits of physical activity in order to help reduce risk factors for chronic disease and manage existing conditions. • Delivers structured exercise programmes tailored specifically to the needs and goals of clients through individual exercise programming and group based activity. • Encourages people of all abilities to become and remain more physically active and help to increase healthy life expectancy, be preventative and act as an early intervention pathway.

  7. HEALTHWISE: HOW TO REFER? The GP surgery will need a service level agreement before referring patients. Please consult your Practice manager, lead GP or senior partners prior to referral. The patient will need to meet the general criteria: • 18Years Old > • A London Borough of Barnet Resident or Registered with a GP Surgery in Barnet. • Considering or ready to make a change (Readiness) and become more active. • Physically Inactive (<30 minutes per week/ Not an existing gym member in the last 6 months) The patient will need to meet the specific criteria for their medical condition/referral reason. Please consult the full inclusion/exclusion criteria on Bar Global or contact Healthwise. OUTCOMES Patient Journey Adult Weight Management 10 Week Programme 12 Week Physical Activity Programme • Group Activity (15 capacity) • 3 Assessments (Week 1& Induction, 6 & 12) • BMI 30+ • Signposted to supervised gym sessions or • 2 hours per week – Conference or meeting rooms, other activities at the centre – can come as gym and studio. many times as they like • Aim: 3 – 5 % weight loss • Burnt Oak, Copthall & Finchley Lido • Full Check & Measurements (Week 1 & 10) • Hendon (Q1 2019) & New Barnet (Q4 2019) • Each individual’s weight measured weekly

  11. Adult Weight Management Criteria • BMI (BAME group 27.5 - 40 or White Caucasian 30 – 40). General Information • Group Activity (15 capacity) • 10 week intervention • 2 hours per week – healthy eating and physical activity • Aim: 3 – 5 % weight loss • 3 Locations: Burnt Oak, Copthall and Finchley Lido • Full Check & Measurements (Week 1 & 10 – BP, RHR, WC, Weight & EQ-5D-5L) • Each individual’s weight measured weekly The patient must not have any of the following Contraindications • Pregnant or breastfeeding • A diagnosed eating disorder • Comorbidity or underlying medical cause of obesity which requires medical intervention • Unstable/Uncontrolled moderate/severe mental health condition
