Physical Activity Referral Scheme
- Enables health professionals to refer patients with a range of medical
conditions.
- Introduces participants to the benefits of physical activity in order to help
reduce risk factors for chronic disease and manage existing conditions.
- Delivers structured exercise programmes tailored specifically to the needs
and goals of clients through individual exercise programming and group based activity.
- Encourages people of all abilities to become and remain more physically
active and help to increase healthy life expectancy, be preventative and act as an early intervention pathway.

- Considering or ready to make a change (Readiness)
- Physically Inactive (<30 minutes per week/ Not an

- Referrers: Barnet Hospital &
- 12 week programme - Cancer
- Exit Route in to Healthwise –
- Direct referrals/indirect
- Specific Pathway – HBa1c
- All Medical conditions that
- Primary Care – GP surgeries
- Referred to Healthwise
- Preventative Pathway – 1 risk

Healthwise supervised Sessions REJECTION Patient does not meet the inclusion criteria for the Healthwise scheme. OUTCOMES ACTION Patient’s referral form is missing or requires more information to process referral against inclusion criteria. UNCONTACTABLE Patient can’t be contacted by the Healthwise team. Letter sent to home address for patient to contact the team. ON HOLD Contraindication to exercise d u e t o u n c o n t r o l l e d symptoms/measurements related to referral reasons ACCEPTED POST GRADUATION 3 Month Follow Up (PARS only) 6 Month Follow Up (AWM & PARS) 12 Month Follow Up (AWM & PARS) Please make sure all information is correct and valid on the referral form prior to referral to prevent delays in processing the referral and disappointment if the patient does not meet the inclusion criteria. Criteria
- BMI (BAME group 27.5 - 40 or White Caucasian 30 – 40).
General Information
- Group Activity (15 capacity)
- 10 week intervention
- 2 hours per week – healthy eating and physical activity
- Aim: 3 – 5 % weight loss
- 3 Locations: Burnt Oak, Copthall and Finchley Lido
- Full Check & Measurements (Week 1 & 10 – BP, RHR, WC, Weight & EQ-5D-5L)
- Each individual’s weight measured weekly
The patient must not have any of the following Contraindications
- Pregnant or breastfeeding
- A diagnosed eating disorder
- Comorbidity or underlying medical cause of obesity which requires medical
intervention
- Unstable/Uncontrolled moderate/severe mental health condition

Healthy Lifestyle Service
What is XPLORE?
- Family Lifestyle Programme aimed at helping children and young
people who are over the healthy weight range and between the ages of 4 and 13 years to move more, eat well and feel good
- FREE 8 week programme with 12 months maintenance that supports
families to make sustained lifestyle and dietary changes
- Referral criteria:
− BMI > 91st centile without complex needs
- Eligibility criteria:
− Live in the borough / registered with a GP and/or attend a school, 6th form college or further education in Barnet
Why its needed?
- Xplore uses the NCMP data to prioritise schools and areas of the
borough to deliver programmes. Key messages for Barnet from latest results (2016-2017)
- In 2016/17, almost one-fifth (19%) of 4–5 year olds and almost one-
third (32%) of 10–11 year olds were either overweight or obese.
- Obesity levels in 10–11 year olds were twice as high as in age 4–5 year
- lds.
- Barnet levels of overweight, obesity and excess weight were
significantly lower than London levels, for both age groups.
- 10 to 11 year olds in the most deprived areas had double the obesity
level of those in the least deprived areas; for 4–5 year olds levels were 50% higher.
