HEALTHWISE: HOW TO REFER?

The patient will need to meet the general criteria:
  • 18Years Old >
  • A London Borough of Barnet Resident or Registered
with a GP Surgery in Barnet.
  • Considering or ready to make a change (Readiness)
and become more active.
  • Physically Inactive (<30 minutes per week/ Not an
existing gym member in the last 6 months) The patient will need to meet the specific criteria for their medical condition/referral reason. Please consult the full inclusion/exclusion criteria on Bar Global or contact Healthwise. The GP surgery will need a service level agreement before referring patients. Please consult your Practice manager, lead GP or senior partners prior to referral. The patient must not have any of the following Contraindications

  • Pregnant or breastfeeding
  • A diagnosed eating disorder
  • Comorbidity or underlying medical cause of obesity which requires medical

intervention

  • Unstable/Uncontrolled moderate/severe mental health condition

What is XPLORE?

  • Family Lifestyle Programme aimed at helping children and young

people who are over the healthy weight range and between the ages of 4 and 13 years to move more, eat well and feel good

  • FREE 8 week programme with 12 months maintenance that supports

families to make sustained lifestyle and dietary changes

  • Referral criteria:

− BMI > 91st centile without complex needs

  • Eligibility criteria:

− Live in the borough / registered with a GP and/or attend a school, 6th form college or further education in Barnet

Why its needed?

  • Xplore uses the NCMP data to prioritise schools and areas of the

borough to deliver programmes. Key messages for Barnet from latest results (2016-2017)

  • In 2016/17, almost one-fifth (19%) of 4–5 year olds and almost one-

third (32%) of 10–11 year olds were either overweight or obese.

  • Obesity levels in 10–11 year olds were twice as high as in age 4–5 year
  • lds.
  • Barnet levels of overweight, obesity and excess weight were

significantly lower than London levels, for both age groups.

  • 10 to 11 year olds in the most deprived areas had double the obesity

level of those in the least deprived areas; for 4–5 year olds levels were 50% higher.

From late 2019 we will be in all of our leisure centres including the new Copthall and New Barnet Leisure Centre.
  • We focus on getting across the borough so also deliver in

primary schools targeting the top 15 priority list from the NCMP results and those in the most deprived areas.
