Socialserve.com data base is the tool this pilot will utilize to connect to housing options The inmate will be matched to the appropriate level of services based on scores provided by ADC This follows what is called the Risk Need Responsivity model of recidivism reduction. This model will differentiate low risk offenders from higher risk offenders in order to provide the appropriate level of treatment

▪13 weeks of housing paid ▪Next 13 weeks are paid by the participant ▪If employed and determined apartment ready, they can be referred to Rapid Re-Housing and be set up in an apartment ▪The expectation would be they sustain the housing for the remainder of their lease timeline
