Visit • Exit Meeting at the end of the day:  Recognize all of your site strengths!  Discuss areas that can be strengthened for re-accreditation.

  8. • HFA now calls the Site Self Assessment (SSA) the Se Self lf-Study. • The specific policy to paste onto Self- Study is listed on the submission guideline for each area. SELF-STUDY • Please wait to paste local policies until REMINDERS you have updated your local PPPM with changes from the feedback form, if applicable.

MANAGER QUESTIONS STAFF RESUMES  Resumes submitted need to be current to reflect the position that the staff is holding at the time of submission .

  11. CONTINUOUS QUALITY IMPROVEMENT Reviewing the standards, assessing our outcomes, asking ourselves and staff questions, improves the quality of the program and the quality of services that families are receiving… It does not mean perfection!

  12.  Do you have comprehensive and up-to-date data regarding your target population?  Have you clearly identified your community partners? Would you be rated a 2 or 3? CRITICAL  Are you following your local screening process listed in your PPPM? ELEMENT  Are at least 80% of your screens occurring either prenatally or within the first two weeks after the 1 birth of the baby?  Are at least 80% of first home visits occurring prenatally or within the first three months after Initiate services the birth of the baby? prenatally or at birth.

  13.  Is the NBQ being completed uniformly at your site clearly showing eligibility for HFO?  When you are at capacity, are you utilizing your CRITICAL service priorities as stated in your PPPM? ELEMENT 2 Use standardized screening to systematically identify and assess families most in need of services.

  14.  Is your site following the procedure of offering families the program on a voluntary basis?  Are you following your local procedure for families involved with Child Welfare? CRITICAL  Is your site utilizing a variety of positive methods to engage families in services, build trust and keep ELEMENT families involved? How would you know?  Are you placing families on creative outreach any 3 time after a first home visit when disengaging from services, conducting activities during the course of creative outreach and continuing Offer services voluntarily creative outreach for at least three months? and use positive, persistent outreach efforts to build family trust.
