Before entering 7th grade, students must have the following documentation in to the nurse: • Physical Exam (dated within a year of entering 7th grade). • Tdap/Tetanus Booster Vaccination • 2nd Chicken Pox/Varicella Vaccination FYI...7th graders have screenings of hearing, vision, postural, height & weight. • Also, SBIRT: Screening, Brief Intervention, and Referral to Treatment begins in grade 7.
