How It Works with the State Registry

Provider-Initiated Patient Registration

Patient completes an AZ MyIR registration form in your office. Your office reviews and approves the application during the visit, matches records to ASIIS, and provides the patient with a temporary password/PIN and a program user guide. The patient logs in at home, uses the temporary password/PIN to activate the account, and creates a new password. The patient now has immediate access to family immunization records.

Consumer-Initiated Patient Registration

Patient pre-registers for AZ MyIR and sets their own password/PIN before visiting your office. Your office reviews and approves the application in person, matches records to ASIIS and gives the patient a program user guide. Patient logs in at home, activates the account and has immediate access to copies of immunization records anytime, anywhere through AZ.MyIR.net.
