Dr Ella Winstanley, Broomwood Surgery
“…I would like to personally thank you for the hard work done by you and your staff at BTSE/Bromley Well, in preparation for, during and following the dispersal of Charterhouse patients into neighbouring GP practices. BTSE/Bromley Well has been invaluable for assisting us with communications to patients, including some of the most vulnerable left on the Charterhouse list. Your staff have contributed their time, energy and goodwill in delivering a safe closure, and patients have been better

Name of Service: Information, Legal Advice & Guidance – Housing Describe the support the client received from the pathway and the difference it made The client presented via an outreach drop-in, having just received an eviction notice giving an eviction date of 10.10.19. The client was advised to update an earlier homelessness application to LB Bromley and was given a Caseworker. The client had tried to keep to the terms of the court order, but when the Caseworker called the landlord they clarified the actual terms of the order, which the client agreed had been misunderstood. The Caseworker completed the court forms to explain the client’s efforts to pay, the misunderstanding and the probable misrepresentation on the
and an alternative order substituted on terms of full rent. The client was also advised on the process to secure an emergency hearing. Explain the outcomes achieved for the client The Court agreed with the case made and eviction was cancelled and homelessness prevented and the client given a more affordable payment arrangement of rent/arrears.

Name of Service: Information, Legal Advice & Guidance - Welfare Benefits What was the client’s situation on first coming to Bromley? Cl was homeless, sofa surfing with her cousin. Cl had no income, as she had previously had an appointee to handle her benefits, who when the relationship broke down failed to pass requests for information on to cl. Describe the support the client received from the pathway and the difference it made Cl was supported by Housing Caseworker (CW) to make a homeless application, but due to nil income no offers of property could be made until her benefit issues were

Cl was supported by Forms Completion Service to complete new PIP and UC-50 forms. Explain the outcomes achieved for the client Appointee was removed from cl’s benefits, cl’s PIP was re-instated and backdated to March 2018 (approx. £9,000 backdated amount), with an ongoing award worth £119.90pw (approx. £15,500 over course of award). Cl was awarded UC at £317pm, and after 3-month wait was awarded additional UC at £336pm ongoing. Cl has now been found a property, with support from Housing CW, and UC has now been updated to include a Housing Element of £657pm. CT reduction has also been received. In total the cl’s income has increased by over £1830pm. Cl’s application to the Bromley Welfare fund, and Homeless packs resulted in the cl being provided with a Fridge/Freezer, Cooker, Kettle, pots and pans, and blankets for her new home. Name of Service: Long Term Health Conditions What was the client’s situation on first contacting the Bromley Well service? Housing Benefits initially and was referred to LTHC service for support with her health conditions. Describe the support the client received from the pathway and the difference it made: Client has various health conditions including hypertension, depression and personality disorder. Client attended the health and wellbeing programme and needed more support with medication on a 1-2-1 after the programme as she forgets to take her medication. Scheduled a face-to-face (1-2-1) with client and discussed tips on improving adherence to medication such as setting an alarm clock and an audio reminder i.e. Alexa, organising tabs/capsules in a pill box etc. Informed client about Bromley Medicines Optimisation Service and provided referral forms in case client would like a home visit from the pharmacist or pharmacy technician for further support. Client set some smart goals to look at different audio medication reminder options and organising her tablets in a pill box. A 2-weeks follow up was scheduled. Explain the outcomes achieved for the client: Followed up on client after 2-weeks to establish any progress made on her set goals. Client has had a look at different audible medication reminders including echo Alexa and is trying to use different phone apps as reminders. Client mentioned that she now feels more confident taking her medication and not forgetting as she used to. She also feels confident in managing her condition. Client has joined a 12-week ADHD management course, which she is enjoying. Client would like to join the sessions she missed for the Orpington workshops. Client is grateful for the support she has received from the service and mentioned that it is always good to speak to someone on a 1-2-1 basis.
