Laynhapuy Health is an Aboriginal Community Controlled Health Service. We cover approximately10,000 km2 In East Arnhem Land, Northern

comprehensive Primary Health Care to over 1000 Yolngu people across 21 remote Laynhapuy Homelands.

Bundle Payment – How it works for Us

The bundle payment provided by HCH has allowed us to improve on our already existing holistic and community driven approach to health and wellbeing. We have been able to expand our health promotion projects, such as community shops, nutrition and cooking programs

Community Stores

The HCH bundle funding has assisted with community health development through supporting our community shops. The main focus of these shops is to improve food security and nutrition. The benefits of access to healthy and affordable food cannot be underestimated for the improvement and management of chronic diseases.

Community Cooking Classes

Cooking classes happen on a regular basis in our larger

  • communities. The classes focus
  • n nutrition advice and utilising

what food is available through the community shops. From a more holistic point of view it has the added benefit

  • f being a social activity,

creating connection and strengthening relationships.

Positive Outcomes for Community

  • Money saved on transport into

town for food. (a bush taxi can cost upto $1300 return from some communities)

  • Yolngu able to stay in their

homelands which means less exposure to some of the more negative aspects of town living.

Scope of Practice

Our model of care has always provided a large scope of practice for Remote Area Nurses as we are visiting remote communities with often very little access to the doctors other than by phone. Majority of what we do is managing the wellbeing of clients with complex chronic conditions. A team based multidisciplinary approach to patient centred care. is the fundamental way Laynhapuy health operates. Health Care Homes model of care supports our current structure and ideology.
