PLAN:

Overall - All Psychosocial clients receiving individual services, complete a psychosocial assessment with a mental health therapist prior to their third visit. Short Term - At least 50% of the psychosocial clients receiving individual services have completed a psychosocial assessment by July 2017. 1) To ensure new PSS Assessment is utilized 2) To ensure the client receives quality care through proper identification and documentation of mental health diagnoses and psychosocial needs.
