Center Accidental Disability Prfsumption (fpnn RS6047-W) or a \Vorld 1):ade Center Notjce (RS6047-N) on or before September 11, 2018. Even if you are no longer in service, if you meet the above eligibility requirements, you may still be eligible to apply fpr this presumption.

If You Are Not Disabled Now

If you meet eligibility requirements 1 and 2, we encourage you lo fjle a Notice on or befpre September 11, 2018. Filing a Notice will protect your rights, and the rights of your benefjciaries to apply fpr benefjts in the future.

If You Have Not Yet Filed For Retirement

If you meet requirements l and 2 and wish to file fpr a disability benefjt now, you must fjle an Application for Wormd Trade Center Acciden­ tal Disability Presumption (fprm RS6047-W). If you do not file for a disability benefit now, you must fjle a World Trade Center Notice (form RS6047-N) with us on or befpre September 11, 20I8 to preserve your rights in the future. These fprms are available on our website at www.osc.state.ny.us0retire or by contacting us toll-free al l-866-805-0990 or 518-474-7736 in the Albany, New York area.

If Ypu Have Already Retired From Public Service

You may apply for a reclassifjcation of your benefit to an accidental disability retire­ ment if you meet requirements I and 2 and you would have been permanently disabled and unable to perfprm yous job had your condition been known and fully developed at the time of your retirement. If approved, your accidental disability retirement will become effective as of the date of reclassification. It is not retroactive to your date of retirement. Please note, your option selection cannot be changed as a result of the reclassification.

HowTo Apply For Reclassification

To apply fpr a reclassification, you must meet the same eligibility requirements as previously stated. You must complete an Application for World Trade Center Accidental Disability Presumption (fprm RS6047-W), have it notarized and send it to the address on the back of this brochure. If you do not fjle fpr reclassification on or before September 11, 2018, you must fjle a World Trade Center Notice (form RS6047-N) by that date to be eligible for reclassifjcation should you become disabled in the future. Ypu can download these fprms fsom our website or contact us by phone to have the form sent to you.

Provisions for an Accidental Death Benefit

Chapter 445, Laws of 2006, provides for an accidental death benefjt fpr benefjciaries of certain retirees who participated in the World Trade Center rescue, recove1y or clean up. Your beneficiaries may convert your retirement ben­ efjt to an accidental death benefjt by filing an Aplication fpr World Trade Center Accidental Death Benefjt (form RS6418-W) if:

  • You fjle a World Trade Center Notice

(form RS6047-N) on or befpre September 11, 2018;

  • You retire under a service or

disability retirement;

  • You have bfen retired 25 years
  • r less at the time of your death; and
  • It is determined that the cause of death

was the result of a ualifzing condition. Only certain benefjciaries (spouse, children

  • r dependent parent) are eligible to receive

this benefjt. Eligible benefjciaries and the time period fpr fjling the application arc the same as those fpr the regular accidental death ben­ efjt provided by your retirement plan. Please consult your plan booklet or contact us for more infprmation regarding this benefjt, including fjling deadlines.
