GP practice registration  Notification form sent to Yvonne Rodney (Specialist Practitioner Immunisation) St Ann’s Valley Centre 0115 8838700 or 07715049298  YR informs PN, GP, HV, PHE, & CHRD (letter & notification form sent)  Practice to check that child has registered before 4 weeks of age (liaise with HV)  Confirm parent/guardian contact details  Inform parent/guardian of need for vaccination & schedule  Record previous Hep B vaccination and immunoglobulin (if given)  Maintain a reg of babies req Hep B vaccination NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] Maintained a register of babies at risk of Hep B within the practice 3

Call/ Recall  Invite child for vaccination as per schedule – -1 mth, 2 mths and 12 mths & at 3yrs 4 mths – 5yrs (with pre school booster) place alerts for subsequent vaccinations • Ensure vaccine available  Book interpreter if required (CityCare)  Inform HV of appointment date  Contact parent/guardian by phone or text prior to appointment to act as a reminder  Actively follow up children who fail to attend clinic for imms by contacting parent/guardian NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] Maintained a register of babies at risk of Hep B within the practice 4

Appointment/ Data Collection  Inform parent/guardian of need for vaccination and schedule  Vaccination should be given under a PSD  Inform CHRD (SystmOne/ Monthly computer print outs) – include date, batch no, name of vaccine, expire date & site given. (1, 2, 12 mths and 3yr 4 mth- 5yrs (sue.curtis@nottinghamcitycare.nhs.uk 0115 883 9689  Reported on the COVER data, showing uptake on a 1/4 and annual basis  Inform HV NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 5

  6. Monitoring and review  Babies at risk of Hep B should be discussed at monthly health visitor/practice meetings, this should be attended by the lead PN or minutes/ actions fed back  HV & GP will ensure where appropriate vaccination is recorded as part of child protection plans  Liaise when a child fails to attend for schedule vaccination, and where appropriate offer outreach vaccinations NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 6

  7. Completion  On completion of the 4 th dose, arrange for the child to have surface antigen blood test (HBs Ag) at 12 months to exclude infection  Any medical concerns or where a child test positive for infection should be referred to Paediatrician NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 7

  8. Payment • Now in GMS contract £7.64 a dose and states responsibility to ensure blood test is taken. • Claim via CQRS from July 2014 • The cost of the vaccine can be claimed via FP34 or FP34PD NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 8
