Chapters must check yes to the below activities and be able to document and verify all information provided.

Each Grade Group must have activities with an average of 50% attendance for the program year. (Between 5-7 Grade Group activities)
Module Evidence Requirements: Chapters provide sign in summary sheets. (Utilize Five Star Form)
2.
Each member has met or is on schedule to meet 1 in 4 Bylaw requirement during the Five Star evaluation period.
Module Evidence Requirements: Obtained from Chapter VP and verified by Chapter President.
3.
At least 25% of chapter teens are registered for the Regional Teen Conference.
Module Evidence Requirements: Regional Secretary provides total # of Teen Conference Registrants and cross reference with total # of Chapter Teen in MIS from HQ. (Average of 2016/17 and 2017/18 program year) (Utilize Five Star Form)

At least 50% of chapter members have served in a Jack and Jill of America, Inc. leadership position (Including grade group coordinator, committee chair, appointed or elected officer).

The chapter has a written membership assessment, sponsorship, and retention strategy.
Module Evidence Requirements: Submit dated material (committee report, event program, or minutes) to show evidence of the membership assessment, sponsorship, and retention strategy.

Module Evidence Requirements: Submit committee report, event program, or minutes to show Chapter executed each of the recognized National Jack and Jill of America, Inc. traditional days of observance, including the date celebrated.

The Chapter has been recognized by the National Program Director (NPD) for a Superior or Excellent Chapter Program Handbook.
Module Evidence Requirements: Submit copy of letter from NPD regarding the Chapter Program Handbook score.
4.
Chapter submitted articles for SCOPE and Up the Hill.
Module Evidence Requirements: Submit copy of email from headquarters verifying submission of Chapter articles for both Jack and Jill of America, Inc. publications.
5.
Chapter has current Liability and Medical Information Release Forms for each child and teen.
Module Evidence Requirements: Submit copy of spreadsheet listing each child/teen with grade group coordinator/lead teen sponsor signature indicating receipt of each child's and teens signed forms. (Utilize Five Star Form)

Chapters must check yes to the below activities and be able to document and verify all information provided.
1.
Executive Board meetings are held prior to all general Mothers meetings and agendas and monthly Treasurer's Report distributed at both meetings.
Module Evidence Requirements: Include a copy of the chapter calendar highlighting dates of Executive Board and chapter meeting dates. Provide ONE (1) sample from each quarter for the program year(s), provide meeting agenda, meeting minutes and monthly Treasurer's Report.

Chapter ceremonies are held according to Jack and Jill of America,
- Inc. protocols.
Module Evidence Requirements: Highlight Jack and Jill of America, Inc. ceremonies held during program year(s) and provide 2-3 photographs (after formal ceremony) confirming appropriate ceremonial set up and proper attire of attendees.

Chapters members receive communications from all levels of the organization (National, Regional, Chapter, Committee, and Grade Group information).
Module Evidence Requirements: Include excerpts or screenshots

documentation sent to validate the chapter communications, examples such as chapter newsletters, weekly emails, etc.

Your chapter has been free of Code of Ethics violations and substantiated grievances for the last three (3) years.
Module Evidence Requirements: Include a letter of notification from Regional Director, which also includes a statement of chapter support.

Your chapter hosted a Regional or National event within the last ten (10) years or is scheduled to host an event as noted on the Time and Place committee report.

Chapter utilizes Right Networks as the platform for hosting QuickBooks.

Chapter submits all Jack and Jill of America, Inc. Financial Reports accurate and on time:

Chapter has an appointed a Foundation Chair.
Module Evidence Requirements: Provide committee/chair listing from program handbook or meeting minutes listing the Foundation Chair.

Each Grade Group in the chapter performs a community service activity during the program year. (Can include chapter-wide community service project)
Module Evidence Requirements: Provide a program, event flyer, email, activity report or minutes of the event held for each chapter grade group service project. (Include no more than ONE photo per grade group activity as added evidence. Include a caption including date/title
- f event and brief description.)

Bonus Assessment Tools (2017-2018 Program Year)
1.
Chapter applies for Jack and Jill of America Foundation grant.
Bonus Evidence Requirements: Chapter must provide letter from Jack and Jill of America Foundation confirming grant application submission.
2.
Chapter has an active Father's Auxiliary Group.
Bonus Evidence Requirements: Chapter must provide a Father’s Auxiliary roster and calendar/listing of Fathers' Auxiliary activities. You may also submit 1-2 photos (maximum) highlighting the Fathers' Auxiliary activities included on the calendar.
3.
At least 25% of the children are registered for Children's Cluster.
Bonus Evidence Requirements: Chapter secretary provides total # of Children's Cluster Registrants and cross references with total # of chapter children in MIS. (Average of 2016-2017 and 2017-2018). (Utilize Five Star Form)
4.
Chapter had representation at On the Hill Legislative Summit.

Assessment Tool Point System
- Each Module has five (5) Assessment Tools for a total of five (5)
possible points per Module.
- ONLY one (1) Bonus point can be applied to supplement/support any
- ne Module.
- Chapters can achieve all four (4) of the Bonus Points.
- A minimum of 25 points equates to the total necessary to attain
Five Star Chapter Excellence Recognition given that the appropriate scoring is achieved in each module.
- Only FIVE (5) Stars will be awarded for the Five Star Chapter Excellence
2.0 Recognition.

Gold Pin for all Five Star Chapter Excellence members
✪
Special seating for Five Star Chapter Excellence members at Regional and National events for two program years.

Chapter name will be added to Five Star Chapter Excellence plaque to be housed at National HQ.

Five Star Chapters will be highlighted on national website and in select Jack and Jill of America, Inc. publications.

Five Star Chapters will be invited to host a minimum of one mentoring activity on achieving Five Star Chapter Excellence.
