  2. Background • Started in April 2014 • Formalised in July 2014 • Standard Operating Procedure Commissioning Quality Care

Process • Monthly downloads of ePACT schedule 2 & 3 Tags • Review of data and highlight queries • Queries reconciled with previous responses • Queries raised in a letter and sent to GP practice • Reminders sent after 10 working days. • Unsatisfactory or no responses received then the escalation process begins. Commissioning Quality Care

  4. Criteria for raising a query Excessive quantities – Exceeding 30 days supply • Unusual Controlled drugs or quantities prescribed • Non-formulary Controlled drugs prescribed • Controlled drugs intended for short-term • Shared – Care Controlled Drugs e.g. Methadone • Commissioning Quality Care

  5. Controlled Drugs Report 2016/17 Reviewed 8544 Schedule 2 drugs • Reviewed 8012 schedule 3 drugs • 1297 queries had been identified • 174 letter sent to the GP practices • Commissioning Quality Care

Common Themes from 2016/17 Palliative care patients • Recommended by St Giles • Started by specialists/Secondary Care/ Pain teams • Complex patients • Tolerance and adherence to previous prescribed • medicines Patients transferring from one GP practice to another • Specialist requesting prescribing doses outside drug • licences Chronic pain relief • Commissioning Quality Care

Prescribing changes during 2016/17 Doses reviewed and quantities changed inline with 30 • days good practice Non-formulary preparations changed to formulary choice • options Pharmacy dispensary errors reported • NHSBA transcription errors • Medicines stopped • Practice nurse non-medical prescribing process • reviewed Commissioning Quality Care
