History of Capabilities • Founded in 1997 by Bill and Karen Blumhorst, a former special education teacher • 11 offices serving over three-fourths of Ohio, making us the largest private provider of career rehabilitation in the state • Six categories of core services: • Community Employment • Community Inclusion • Driving Services • Exploration Services • Professional Training • Technology Services

What Sets Us Apart? • No longer facility-based – fully in the Dayton area communities • Transportation is provided by the Community Consultant who works directly with Club members • Members typically work with the same Community Consultant • Provides consistency • Encourages person-centered service delivery • Expansive variety of community sites and connections • Offers options the members can choose from to plan their day • Blend of volunteering and recreation

  5. What Does a Typical Day Look Like? • Morning pickup via Capabilities van or small bus • Pickup other Club members • Spend the day volunteering (ex. Dayton Food Bank ) or a recreational activity (ex. UD Flyers basketball game ) • Return home via Capabilities van or small bus

How Do We Measure Success? • Lives are enriched through our Clubs in many ways aside from vocationally • Improved interpersonal skills through socializing with friends • Experiencing different hobbies • Discovering new interests • Making more connections in the Dayton community • Learning how to make informed choices
