The transition period can be a stressful time for people with a learning disability and for their parents and carers. This is because they are usually unsure about the choices and opportunities available. It is however, a time to plan for the young person’s future and a time to discover what opportunities are available for a young person on the road to independence.

Age 14 The year 9 Transition Review is held at

  • school. Educational advisors (previously

Connexions) and the school advise on further education. If attending, Disabled Children’s Team advise on potential future services or current services required. Discussion starts whether the young person is likely to meet the criteria for Adult Services and if so, which Adult Team is appropriate.

Age 16 Year 11 annual review – The Transition Team will aim to join the school review. Looking to obtain information about service required age 18, further education, housing needs, day care possibilities etc

Age 17.5 Transition Team complete Community Care Assessment (CCA) to identify eligible and non-eligible needs under Fair Access to Care Services (FACS) (see 1.4 Eligibility). Respite options to be discussed. !! Respite at Hollybank stops on the day before the young person 18th birthday!!
