  11. Credentialing is the verification and approval of a practitioner's credentials. • Did they go to school? Where? When? • Did they obtain their degree? • What is their employment history and in what capacity? • Have they been involved in any malpractice cases? What was the outcome? Etc., etc., etc., etc. Top 4 Plans Identified 1. Independence Blue Cross Blue Cross/Blue Shield ✓ Primary focus – Original/Traditional 2. Aetna Medicare 3. Humana ✓ Need to expand beyond original Medicare 4. United Healthcare ✓ Analysis of Medicare Advantage Plans * Addition of Tricare VA Plans

  12. Three Steps: 1. Practice application 2. Medical Director/Physician application 3. NP application ✓ 15 +/- pages in length ✓ Multiple attachments ✓ 3 to 6 months to completion of each application ➢ Can take up to 18 months for final approval for 1 NP for 1 insurance carrier

  13. 15 applications per practice x 27 NPs = approximately 400 + applications to complete the project for the 5 identified Medicare Advantage plans ✓ Internal staff ✓ Outside credentialing agency

  14. ➢ At least 30% of time at work ➢ Work place – logical place for employees to receive services Supports: ✓ Healthy working environment ✓ Fosters healthy lifestyles ✓ Preventative health ✓ Quicker intervention for illnesses, reducing absenteeism and missed working hours

  30. Medicare Certified Skilled Home Health • January 2017 - Acts Home Health in PA was Accredited by The Accreditation Commission for Healthcare, Inc. (ACHC) • Medicare Certified Home Health Services in 8 PA Communities • Includes Skilled Nursing, Acts In-house Rehab for Physical, Occupational and Speech Therapies, & Home Health Aides • Contracts with Aetna and more recently with IBC/BS for Keystone 65 and are working to secure other Medicare Advantage HMO providers.

Acts Signature Hospice • Acts Signature Hospice serves all 8 PA Communities and two DE communities • Served over 233 residents and their families in 2018 • Increased the average length of service from 65 to 97 days • Provided over 21,034 total days of care • Residents may receive hospice services in all levels of care

Acts Signature Hospice • 70+ actively engaged Acts Residents serving as Hospice Volunteers - 16 Hours of Training required. • We are identifying, celebrating and empowering Hospice Champions in each Community we serve. • Added services: We Honor Vets, Last Wishes, Complementary Therapies (massage, reiki, music), Virtual Goggles, and an Interactive Robot Cat Companion, especially for those living with Dementia.
