Interdisciplinary Teams

  • Funding to support capacity and infrastructure of

interdisciplinary teams to coordinate in addressing non- clinical and clinical needs for low-income and vulnerable populations

  • Goal: Demonstrate that this interdisciplinary team model can

devise a means of coordinating services that leads to:

  • Increased health care access and improved health status for clients;
  • Stabilization or improvement in social needs;
  • Cross-sector collaboration; and
  • Development of standardized non-clinical and clinical metrics that

measure the effect of their efforts

The Grant Program - Basics

  • Funding amounts are up to $75,000 for a planning year

grant to support the development of interdisciplinary teams consisting of social service and health care partners

  • Focus during planning year:
  • Develop propose team infrastructure
  • Workflows
  • Scope of service to clients
  • Develop way to measure and evaluate impact, with support and

technical assistance from Foundation

Interdisciplinary Teams – Who Can Apply?

  • Lead organization (applicant to the Foundation) must:
  • Be a social service organization that provides supports related to a social

determinant of health

  • Provide case management services
  • Have an existing client base of low-income people
  • Demonstrate experience in facilitating initiatives among multiple partners
  • At the point of submitting an application, lead organizations should

demonstrate an existing relationship with at least one other

  • rganization
  • Teams should ultimately consist of at least one health care
  • rganization, and any number of social service organizations
  • Health care organizations (community health centers, hospitals/hospital

programs) are not eligible to apply as the lead applicant; can be a partner within a team

Outcomes/Deliverables of Planning Year

  • At the client level:
  • Description and number of clients served, and the types of social

and health care services that will be addressed

  • A clear workflow to address the needs of clients
  • At the organizational level:
  • Team partners solidified and governance document established
  • Prepared job descriptions or clear plans for how existing staff will

fulfill responsibilities

  • Roles and responsibilities of the partners in serving clients
  • Identification of a data platform to exchange information on clients
  • Regular meeting structure among partners

Reporting, Sharing Best Practices, Ongoing Learning

  • Grantees expected to provide regular reports on the impact of

their work – structure, questions, and timeframes to be dictated by the Foundation

  • Data will be gathered and shared back with grantees and other

stakeholders on trends, lessons learned, and best practices

  • Regular grantee convenings via Learning Community
  • Foundation shares impact and lessons with the field, including

policy issues or recommendations

Reporting & Learning (Continued)

  • Foundation’s consultant will work with grantees to identify and

develop a common set of non-health and health metrics that can be:

  • Used as core measures for the grantees in reporting, and
  • Potentially serve as a universally adopted set of metrics beyond this grant, to measure the

success of interventions that span social services and health care

  • Shared with state level agencies and payers for consideration
  • Enhanced learning provided by Foundation to grantees to develop

knowledge between and among sectors
